As he begins to climb said building, Kessler mentions how Trish is a special woman and wishs that she didn't have to die. As Cole reaches the top of the building to saves the doctors, Trish’s bomb detonates, sending her plummeting to the ground below screaming his name.

Too stubborn and selfish to say goodbye to the woman he loves, Cole quickly scrambles up the building where Trish hangs. As he does this, Kessler calls Cole out on his extremely selfish decision. As Cole reaches the top of the building he learns that Kessler had replaced Trish with another woman, and placed Trish in the group of doctors on the other building. Before he can scream foul, the real Trish and the doctors fall to thier deaths.

No matter which choice Cole makes, Trish falls to her imminent death. Using what's presumably [[Pulse Heal]], Cole is able to revive her for just a few moments and depending on which side of the Karma Meter hes on, she will tell him one of two things;

If Cole is good, Trish tearfully tells him that she was proud of his actions and all the positive choices made for the city. Happy to see all the good he had done, as Trish breathes her final breath, she tells Cole with a smile on her face that she loves him.

If Cole is evil, during Trish's final moments, she tells him that shes ashamed of what hes become. As she lays there breathing her final breaths, Trish scorns Cole for squandering the powers God gave him by using them to hurt others and think only of himself. Trish then dies bringing her hate for Cole with her to the grave.
